Statement of Economic Interests – Form 700

The Political Reform Act (Gov. Code Section 81000-91014) requires most state and local government officials, consultants, and employees to publicly disclose their personal assets and income. They also must disqualify themselves from participating in decisions that may affect their personal economic interests. The FPPC is the state agency responsible for issuing the Statement of Economic Interests, Form 700, and for interpreting the law’s provisions.

Per CA Govt Code § 81008 (2022), copies of Statements of Economic Interests (Form 700) can be obtained from MTS’s Clerk of the Board by filling out this form. Alternative format requests for the inspection or reproduction of reports and statements are available upon request.

Conflict of Interest Code

The Political Reform Act, Government Code Sections 81000, et seq., requires government agencies to adopt and promulgate a Conflict of Interest Code. The most recent version of the Conflict of Interest Code lists positions who have been designated to disclose their financial interests.

Payment to Agency Reports – Form 801

This form is used to report certain payments received by state and local government agencies. It includes:

a payment for an official’s travel expenses for the purpose of facilitating the public’s business in lieu of a payment using agency funds; and
a payment that would otherwise be considered a gift or income to the benefiting official, but is instead accepted on behalf of the agency.

FPPC Regulations 18944 and 18950.1 provide a procedure that state and local agencies may use to disclose payments used for agency purposes and paid by a third party. The regulations’ reporting procedures provide an alternative means to disclose a payment that may otherwise be considered income or a gift to a benefitting employee and subject to reporting on a Statement of Economic Interest, Form 700.



Agency Report of: Ceremonial Role Events and Ticket/Admission Distributions – Form 802

This form is for use by all state and local government agencies. The form identifies persons that receive admission tickets and passes and describes the public purpose for the distribution.

FPPC Regulation 18944.1 sets out the circumstances under which an agency’s distribution of tickets to entertainment events, sporting events, and like occasions would not result in a gift to individuals that attend the function. FPPC Regulation 18942 lists exceptions to reportable gifts, including ceremonial events, when listed on this form.



Agency Report of Public Official Appointments – Form 806

An official may vote to hold another position even if the voting official will receive additional income, so long as the public is informed prior to the vote or consent item. FPPC Regulation 18702.5 requires the official to file Form 806 under these circumstances. When there is a change in compensation or a new appointment, the Form 806 is updated to reflect the change. The form must be updated promptly as changes occur. Each agency must post on its website a single Form 806 which lists all the paid appointed positions to which an official will vote to appoint themselves.
